Hospital Admission Risk Program (HARP)

In collaboration with Eastern Health, Whitehorse Community Health Service is conducting a program specifically designed to assist people with heart failure to understand and manage their health better.

The goals of the program are:

  • To assist you with improvement in your physical and psychological well-being through a multidisciplinary team approach.
  • To provide an education program to both you and your family that helps you to understand your disease and its ongoing management.
  • To provide a graduated exercise program that assists you to become physically active at a level compatible with the function of your heart.
  • To encourage people with heart failure to become active participants in the management of their disease.

Eligibility

Referral to attend the program is required from your GP or Cardiologist—contact the Heart Failure Case Manager at the Box Hill Hospital ph 9895 3333.


Fees

The total fee is $48.00 per program ($36.00 concession). This is to cover the cost of catering and educational materials. Fees may be paid weekly at each session or in full at the first session. Fees can be waived in situations of financial difficulty. Please discuss with program coordinators.
