Please note that in the case of dental services, your information may be accessed by both School Dental Services and Dental Health Services Victoria. Please note that WCHS has a procedure on how to ‘Access your Information’. It is a simple process that involves you discussing the request with a manager. On most occasions you can either come into the centre to view your client file or you can be provided with a photocopy of the information you require. Only in exceptional circumstance will access to your information be denied. (This includes: where access will interfere with legal proceedings or where access would constitute a serious threat to the life or health of another individual). |
